Passport

What is Passport Funding?

Passport is a reimbursement program funded by the Ministry of Children, Community and Social Services (MCCSS). It assists individuals who are 18 years or older with a developmental disability to access services and supports within their communities. The program is also an aid to unpaid caregivers of those with a developmental disability, to obtain respite services and supports.

How to Apply?

The first step is to call Developmental Services Ontario (DSO) at 1-855-277-2121 or by email at dsocentraleast@yssn.ca to request services. A Service Navigator will advise individuals of the process and the documents required to ensure eligibility. Once confirmed and the DSO application package has been completed and processed, the individual will be added to the Passport Program waitlist.
