Our Transitional Age Youth Services (TAY) provide specialized transitional planning for youth who are funded by the Ministry of Children, Community and Social Services to prepare the individual, their family, and their team for the transition from child to adult-funded support and services. TAY Coordinators work to ensure services and supports meet legislative requirements and work in collaboration with the individual’s circle of care to create service opportunities. TAY Coordinators also work to profile youth issues and gaps in service through committee membership, focus groups, and training events.

Who is eligible?

To be eligible for TAY services, an individual must:

  • Be 14 – 21 years old

  • Have a referral from the Child in Care Worker or a Regional Complex Care Case Manager from one of the following agencies:

    • Durham Children’s Aid Society

    • Highland Shores Children’s Aid society

    • Kawartha Haliburton Children’s Aid Society

    • Catulpa Community Support Services

    • Dnaagdawenmag Binnoojiiyag Child & Family Services

    • Ministry of Children, Community and Social Services

How do I access services?

Referrals to TAY can only be made by the listed agencies. For more information, please contact our Central Navigation team.

  • When you call Central Navigation, they will answer your questions and if applicable schedule you an intake appointment with a Specialist. The intake will take place over the phone and last approximately 45 minutes. The specialist will ensure they gather the relevant information to match you to the relevant Canopy Services.
