Our primary service area extends across five regions – Northumberland, Peterborough, Kawartha Lakes, Durham, and Haliburton – and is expanding virtually and physically to reach new areas.

Programs
& Services

  • Adult Case Management is a collaborative, inclusive, and individualized process. We work towards goal setting, planning, advocating, and exploring available community resources to support the achievement of personal outcomes.

    ACM services provide ongoing specialized case management support and oversight within Ontario. Supports include regular communication and check-in meetings with service users and service providers to ensure identified goals are met and adjusted to changing support and administrative needs. Financial oversight of individualized funding as well as submission of service user invoices and expenses is also provided along with the maintenance of multiple legislative requirements.

  • Our Adult Protective Services Program (APS) strives to support individuals to live their lives to their fullest potential, reflecting their values and choices. The APS Program provides case management support and guidance to adults with a developmental disability who are living on their own in the community so they can live as independently, safely, and securely as possible. Our Adult Protective Service Workers (APSW) work directly with participating* individuals to help them access community-based services and supports that increase independence, employment, volunteer and recreational opportunities, and inclusion in the community.

  • Item dOur Adult Urgent Response Program (AUR) provides support, resources, and advocacy for individuals in urgent situations to prevent the further escalation of risk, to mitigate any untenable risks, and to address an unmet urgent need. This case management is a temporary, short-term response or bridging support until a longer-term support plan and/or solution for the individual can be developed.

  • Canopy Support Services has been providing Autism Services to individuals and families in Northumberland, Peterborough, the City of Kawartha Lakes, and Haliburton for more than 20 years. Our dedicated and highly experienced staff deliver family-centred and evidence-based services and supports based on the principles of Applied Behaviour Analysis (ABA). Autism Services are provided by qualified clinicians. All behavioural interventions are overseen by Certified Behaviour Analysts (BCBA). These services are aimed at helping children and youth reach their highest potential.

  • Our Behavioural Consultation Services (BCS) program is designed to enhance the overall quality of life and independence of children, youth and adults with intellectual disabilities by helping caregivers manage exceptional behaviour difficulties that are affecting an individual’s daily life. Using the principles of Applied Behaviour Analysis (ABA), our Behaviour Consultants work collaboratively with families and community partners to deliver behaviour intervention supports in line with the individual’s goals.

  • The FASD program strives to provide support and community connections to children and youth with FASD and their caregivers. At Canopy, the FASD coordinators work directly with the child/youth and their caregiver to set goals, connect with & advocate for other services in the community, and foster parent-to-parent connections. The FASD Program works in the community to provide education & support strategies and collaborates with partner agencies to provide wrap-around support.

  • Funded by the Ministry of Children, Community and Social Services, the Passport Program helps adults with a developmental disabilities be involved in their communities and live as independently as possible by providing funding for community participation services and supports, activities of daily living, and person-directed planning. Passport also provides funding for caregiver respite services and supports for primary caregivers of an adult with a developmental disability. Funds are provided on a reimbursement basis.

  • Our Transitional Age Youth Services (TAY) provide specialized transitional planning for youth who are funded by the Ministry of Children, Community and Social Services to prepare the individual, their family, and their team for the transition from child to adult-funded support and services. TAY Coordinators work to ensure services and supports meet legislative requirements and work in collaboration with the individual’s circle of care to create service opportunities. TAY Coordinators also work to profile youth issues and gaps in service through committee membership, focus groups, and training events.
